I want to
wear my niqaab as normal in ihraam. That will be for umrah upon entering Makkah
and then a second time Ihraam for the days of Hajj. I will not manage with
those caps that try to keep the niqaab away from the face. I find it too
cumbersome.

I know that
there's a Damm penalty for niqaab in Ihraam for more than 12 hours.

A) do i
need to do 2 separate penalty Damm for the 2 seperate occasions of Ihraam (I.e.
entry umrah and days of hajj) or only 1 Damm suffices for both times?

B) Is this
Damm only allowed to be done in Makkah Shareef or can I do it any other country
in the world like a qurbani sponsorship with intention of Damm? 

Answer: 

Firstly, if one commits any act that is contradictory to the state of Ihram, some type of compensation is due on him/her. However, if one indulges in these acts intentionally without any valid reason, he/she will be sinful even after discharging the compensation, since they were done deliberately, and he/she will have to repent with the complete determination of abstinence and avoidance. (Shaami Vol. 2 Pg. 544) 

In other words, will one go and deliberately burn himself/herself or will he/she purposely cut himself/herself just because he/she possesses such an extraordinary ointment which heals and mends rapidly? Just as this (burning or cutting himself/herself based on one possessing an extraordinary ointment) is not an act of intelligence; similarly, indulging and getting involved in the prohibited acts (on the basis that one can fulfil the duty by the expiations) is also not an act of intelligence. 

Moreover, one will be highly rewarded for enduring the difficulties one faces during Hajj & Umrah in the state of Ihram. 

We conclude from above:

It is necessary to put veil (Niqaab) in such a way that the face does not remain covered i.e. by using caps, hats, etc.; however, if one covers his/her face in the state of Ihram for 12 hours or more, Damm (goat/sheep) will be due upon him/her and in the event where one covers less than 12 hours, Sadqah (Sadqat ul Fitr amount) will be due upon him/her. (Kitabul Masa'il Vol. 3 Pg. 180) 

a. 2 Damm will be Wajib (or two sadqas whichever applies), since there are two Jinayat (offences) of two different Ihrams. (Kitabul Masa'il Vol. 3 Pg. 155) 

b. One is required to carry out Damm in the Haram. (Kitabul Masa'il Vol. 3 Pg. 149)
