If one
performs umrah and thereafter without coming out of ihraam he performs another
umrah, he did not
make halaq after the first umrah.

Is there a
penalty?

Answer: 

Yes, a Damm will be applicable for combining two Umrahs (in one Ihraam.)

If a person after completing the Sa’ee of the first Umrah, before removing his hair, ties the Ihraam of a second Umrah, the second Umrah becomes obligatory upon him, and neither of the Umrahs should be discarded, meaning both should be completed. However, a Damm will be obligatory for combining both the Umrahs in one Ihraam. The hair should only be removed after completion of the second Umrah to avoid a further Damm being applicable.
