If the piercing is tight, one should move the ear rings, nose rings, etc., so that the water reaches in the holes and if there are no ear rings in the piercing, then it will be necessary to make the water reach the holes by merely rubbing water over the holes; however, it is unnecessary to insert anything i.e. stick, needles, etc. in the holes to open them. (Shaami Vol. 1 Pg. 153) 

If there is something like food stuck between the teeth which does not stop the water from reaching the teeth, the Ghusl will be valid; however, it is better to clean it, if possible. (Shaami Vol. 1 Pg. 153)
